Youth Individualised Support Program: Funded by the NSW Department of Communities and Justice under the Community and Family Support (CAFS) Program


About the Program

The Youth Individualised Support Program at Volunteering Central Coast provides professional case management and support to young people aged 12–18 years living in the Gosford region.


Support is delivered by qualified Youth Caseworkers who work directly with young people and their families. Our program focuses on improving safety, wellbeing, and long-term outcomes by supporting young people to build independence, confidence, and connection to their community. We use a trauma-informed, person-centred approach that prioritises each young person's voice, strengths, and goals.

What We Do

Young people are supported through tailored, one-on-one case management, which may include: 

  • Intake and comprehensive assessment 
  • Individualised case planning and goal setting 
  • Emotional wellbeing and resilience support 
  • Advocacy and assistance navigating services (e.g. housing, Centrelink, education) 
  • Support with school engagement and community connection 
  • Employment readiness support (e.g. résumés, pathways) 
  • Referrals to specialist services 
  • Family support and capacity building 
  • Safety planning 
  • Mentoring and skill development 

Support is typically provided through weekly or fortnightly sessions, either at school, in the community, or other suitable locations.

Who Can Refer

Referrals can be made by: 

  • Young people (self-referral) 
  • Parents and carers 
  • Schools and education staff 
  • Community services and organisations 

We work collaboratively with local services to ensure young people receive holistic and ongoing support, even after exiting the program.

Outcomes for Young People

Through participation in the program, young people may experience:

  • Increased safety and stability 
  • Improved mental health and wellbeing 
  • Stronger relationships with family, peers, and school 
  • Greater confidence and independence 
  • Increased engagement in education or employment pathways 
  • Improved ability to manage challenges and adversity
